: Indian nationals residing in Thailand can apply to change or correct their name in the passport. This service is processed as a miscellaneous passport service and requires submission of relevant legal documents.

How To Apply

  1. Create an Account: Register on the Du Digital Indian Consular Application Centre (ICAC) website.

  5. Visit Du Digital ICAC: Visit the designated ICAC. The address and location will be mentioned in your appointment letter. Ensure you arrive 15 minutes before your appointment time.
  6. Submit Your Application & Pay Fees: Submit your documents and pay the applicable fees at ICAC. Refer to the Fee Details section for exact charges.
  7. Enroll for Biometrics (if required): Complete biometric enrollment if applicable.
  8. Application Submission Complete: You’ve successfully submitted your application.

Change in Name/Surname for Adults (Above 18)

  • Duly filled Application form
  • Registration form
  • In case one of the parents is deceased, separated, or divorced, please complete the Annexure-C form.
  • Annexure-D
  • Affidavit for change in name – 2 filled copies
  • Court affidavit for name/surname change from India, attested by Notary
  • Cover letter explaining reason for name/surname change
  • Attach Indian ID documents – Aadhar card, Voter ID, PAN card, Driving License
  • Photographs – 2 (2×2 inches, white background)
  • Passport copy – 3 copies each of front page, current Thai visa, last page, and observation page (self-attested)
  • Thai visa supporting documents as per the visa category
  • Advertisement for name change published in Indian and Thai newspapers (submit original newspapers) – Wait 1 month post advertisement before applying
  • If spouse name not endorsed:
    • If married in India – attach marriage certificate copy
    • If married in Thailand/other countries – translated and MFA-attested marriage certificate
    • Spouse’s passport copy – 1 set
    • If Thai spouse changed name/surname – certificate must be translated and MFA attested
  • If married to Thai national – provide copy of Freedom to Marry/Bachelor’s Certificate
  • If address changed in India – provide supporting proof
  • If passport lost – FIR + English translation + cover letter explaining the incident
  • If passport damaged – provide cover letter stating how it got damaged
  • If applicant is not submitting in person – attach authorization letter and ID of authorized person
  • All original documents must be brought for verification at the time of submission.

Yes. You must apply for a re-issue of passport if there is any change in your name—whether due to marriage, divorce, legal name change, or personal preference.

2. What documents are required for a name change in the passport?
The required documents depend on the reason for the name change:

  • After marriage: Marriage certificate and spouse’s passport copy.
  • After divorce: Divorce decree.
  • Legal name change: Gazette notification or name change certificate from a competent authority.

3. Is personal appearance necessary for submitting the application?
Yes. You must visit Du Digital ICAC in person with a prior appointment to submit your application and original documents for verification.

4. Can initials or abbreviated names be used in the new passport?
No. The passport system does not accept initials or abbreviated forms (e.g., SK Singh, KNR Nair). Full expanded names must be provided. Titles such as Mr., Mrs., Dr., Shri, or Late should also not be used.

5. How should the new name be written in the application?
The new name should be entered in block capital letters, fully expanded without prefixes or honorifics. Ensure consistency with supporting documents.

6. Can I apply for re-issue online for a name change?
Yes. You can register and fill the form online, but the submission of documents and biometric process must be completed in person at Du Digital ICAC.

7. Will my passport number remain the same after name change?
No. A new passport booklet will be issued with a new passport number, but the old passport number will be referenced inside the new one.

8. What happens to my current passport after re-issue?
The old passport will be cancelled and returned to you. Valid visas on the old passport remain usable as long as you carry both passports while traveling.

9. How long does the process take?
The processing may take up to seven weeks. You will receive SMS alerts during the process and can track the application via the “Track Application” page.

10. Is this process applicable to all name changes, including minor corrections?
Yes. Any change in personal particulars—whether major or minor—requires a re-issue of passport with supporting documentation.
